Person-centred care in stroke rehabilitation and vascular dementia prehabilitation

This project aimed to explore the known care pathways for stroke rehabilitation and preventative measures for vascular dementia. We looked at current literature and current care pathways and resources in NHS trusts to explore possibilities and limitations. This allowed for realistic recommendations for combined stroke rehabilitation and vascular dementia ‘pre-habilitation’ care pathway

Background 

Post stroke vascular dementia or cognitive decline may affect up to a third of stroke survivors in the months or years following incidence of stroke. There is typically an overlap of vascular risk factors contributing to stroke, recurrence of stroke and vascular dementia/cognitive decline. However, the potential for and overlap in care pathways for stroke rehabilitation and the slowing/prevention of vascular dementia have not yet been explored in a way that can be implemented.

Project Aims

  • To identify aspects of stroke rehabilitation that may contribute to preventative measures in vascular dementia
  • To explore how elements of a person-centred care framework can be implemented within existing NHS stroke and vascular dementia services
  • To examine how cultural and organisational contexts influence person-centred care and care pathway design
     

Project Activity

  • Meta-ethnography review, including contextual background
  • Public involvement activities with lived experience experts

Anticipated project activity

  • Co-production of an adaptable training document with NHS stroke teams and stroke survivors with a vascular dementia diagnosis
  • Progression of understanding of preventative measures for vascular dementia, resulting in an intervention to be observed
