The Microsoft Form version can be found by clicking here and can be duplicated and used for your own purposes to collect responses to the feedback form. You or your organisation will need access to Microsoft Forms for this (if you do not have access to Microsoft Forms, you can use still download the word document). When you click this link, you will see ‘Duplicate’ at the top of the page. Click ‘Duplicate’ to open the form in your own Microsoft Forms account, you will be able to edit the form to tailor it to your PPI Group.
