Person-Centred Care in Stroke Rehabilitation and Vascular Dementia Prehabilitation

This project aims to explore currently known care pathways for stroke rehabilitation and preventative measures for vascular dementia. We will look at current literature and current care pathways and resources in NHS trusts to explore possibilities and limitations. This will allow for realistic recommendations for combined stroke rehabilitation and vascular dementia ‘pre-habilitation’ care pathways

Background 

Post stroke vascular dementia or cognitive decline may affect up to a third of stroke survivors in the months or years following incidence of stroke. There is typically an overlap of vascular risk factors contributing to stroke, recurrence of stroke and vascular dementia/cognitive decline. However, the potential for and overlap in care pathways for stroke rehabilitation and the slowing/prevention of vascular dementia have not yet been explored in a way that can be implemented.

Project Aims

  • To understand the crossover in stroke rehabilitative care that could contribute to preventative measures in vascular dementia
  • To understand how elements of a person-centred care framework can be implemented within current NHS resources in stroke and vascular dementia care pathways
  • To understand what elements of a person-centred care framework mean in different cultural and organisational contexts and how this alters potential care pathways
  • To adapt a person-centred care plan, which can be successfully implemented across different cultures and organisational contexts
  • To develop a person-centred care plan for stroke rehab/vascular dementia ‘prehab’ that can be used in practice in different cultural and organisational contexts

  • Co-production of an adaptable training document with NHS stroke teams and stroke survivors with a vascular dementia diagnosis
