Survey on end of life care in the community
CPFT staff caring for older people with severe frailty in the community are invited to take part in an online survey, to share what you think are the most important palliative care needs.Responses will help improve support for community services to deliver care in people's final years of life to ensure that they are comfortable, with all their needs met at home or in residential care settings.
Please read the study information sheet and visit the survey to take part by Friday 27 November.
